New Zealand Visitor Visa Indian Passport

My husband and I are planning to apply for New Zealand visa. We have Indian passports residing and working in US.

I read online they need cover letter hence I need help understanding what all should be included in cover letter(to avoid rejection) and list of documents to be submitted.

Also should we be doing just one application as a couple or individual application per person?
