Yet another form doubt :(

My birth certificate has my nickname since my name wasn't decided then. We announced the change in the newspaper later. Last year I clicked 'No' on the name change option in my OTR, But now that theyve asked about details of birth, I'm worried if this will cause issues later? Oh, Im 22F, just saying
