Defenders not marking tight?

Every game I lose I concede in the same way - whenever I lose possession the opponent just drives a single ball to the striker and my defenders are either standing way off them, or the through ball puts them through on goal.

When I try and attack, I have to fight for my life to get an ounce of space, and I can only get throughballs through if the defensive line is way open. Forget playing to feet, my strikers are already in the CB's arms by the time I go to pass.

So what's the difference? Is it just every single opponent spamming high press whenever they lose possession? There's something I'm missing cause my opponents are so consistent in both their tight marking defence and ability to 2 pass onto goal from CB > CM > ST almost every time.
